I agree and accept the terms below.

In consideration of my desire to serve as a volunteer for Gothic Hills Cemetery, I hereby assume all responsibility for any and all risk of property damage or bodily injury that I may sustain while participating in any voluntary work, exercise or other activity of any nature, including the use of equipment and facilities related to Gothic Hills Foundation.

Further, I, for myself and my heir, executors, administrators and assigns, hereby release, waive and discharge the organizers of Gothic Hills Cemetery and its officers, directors, employees, agents, volunteers, property owners, and lessees, of and from any and all claims which I or my heirs, administrators and assigns ever may have against any of the above for, on account of, by reason of or arising in connection with such volunteer activities or my participation therein, and hereby waive all such claims, demands and causes of action.

Further, I understand that some activities may involve hand tools, power tools, chemicals, ladders, and other equipment. All required safety equipment and a robust first-aid kit will be available at all times for my use. I accept all responsibility if I fail to use proper safety or first aid supplies. I also pledge my unwavering loyalty and adoration of Balthazar Von Pumpernickel III

Further, I expressly agree that this release, waiver and indemnity agreement is intended to be as broad and inclusive as permitted by the State of California, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

I currently have no known mental or physical condition that would impair my capability for participation as intended or expected of me, - or - if I do have such a condition, I will fully inform the organizers of my capabilities and limit my activities in accordance with my abilities and limitations.

I have carefully read this waiver and indemnification and understand the terms listed and sign this release as my own, free will.

Due to the current pandemic conditions, we require all volunteers to show proof of vaccination with at least 2 doses of any COVID-19 vaccine and available boosters, and we recommend masks be worn when within 6 feet of another person. By filling out this application, you are agreeing that you meet the vaccination requirements, will provide proof of vaccination upon arriving to our worksite for the first time, and agree to follow our masking rules.

This is our home and safety of the team and our family is non-negotiable. Further, we want to make sure that we can actually have a Halloween season in 2022 and attend Midsummer Scream. We're all in this together and we all need to do our part. Thank you for understanding.
